Forms Management

The Form Management Module is a versatile tool designed to create, manage, and deploy various types of forms—from surveys and evaluations to quizzes and polls.

New: generate questions with AI

The Forms module now includes a Questions JSON editor built to work with AI models. Download the structure file, ask any AI model to generate your questions in bulk, then paste the result and insert everything into Gohorto with one click on Apply and save. See Questions JSON Editor.

1. Creating a New Form

Click the "Add New" button to start the creation process: - Form Type: Select from Survey, Evaluation, Quiz, or Poll. - Multilingual Support: Define the form name and description in multiple languages. - Visibility Settings: Determine access restricted to registered members, specific projects, link-only access, or public availability. - Notifications: Set specific recipients to be notified for each submission.

2. Question Builder

Build your form using the red "Plus Icon" button: - Sections: Divide lengthy forms into manageable parts. - Question Types: - Single/Multiple Choice (with drag-and-drop reordering) - Paragraph & Text Input - File Input & Signature - Matrix questions - Validation: Set mandatory fields, unique questions, or specific input patterns (dates, numbers). - Grading: Assign weights and scores to questions for automatic grading in quizzes and evaluations. - Questions JSON: On an existing form, open Actions → JSON to insert AI-generated questions in bulk, or edit every question, language, choice, and score in one view, then Apply and save. 3. Deployment and Responses

  • Preview: Test the respondent experience before publishing.
  • Publishing: Generate a unique URL for sharing via email, web embeds, or other channels.
  • Managing Replies: Access the "Replies" tab to view detailed answers, filter responses by date/name, and export data to Excel or CSV.
  • Visual Reports: Use the "Report" action to view graphical representations of data, such as pie charts and bar graphs.
